Form Usage

Use this form to confidentially report any crime, safety issue, fraud, or general concern. You can submit reports such as:

  • Crime & Safety: Active crime tips, property theft, identity theft, or campus/facility safety hazards.
  • Ethics & Compliance: Financial aid fraud, workplace misconduct, regulatory non-compliance, or ethical violations.

Anonymity & Monitoring: Reports are completely anonymous by default unless you choose to share your contact information at the end of this form. We review this site during regular weekday business hours and not 24/7.

If you are in physical danger, are reporting a crime in progress or an active emergency, call 911, local emergency services or NMU Police Dispatch at 906-227-2151.

Due to a nationwide surge in "ghost student" financial aid schemes, NMU has a dedicated team investigating financial aid fraud reports.

If you received an unexpected 1098-T tax form, financial aid notice, or welcome email from NMU without applying, your identity may have been compromised. Select "Identity Theft / Unauthorized Financial Aid Application" under Type of Concern for immediate routing to our financial aid fraud team.

Required: Examples include Identity Theft / Unauthorized Financial Aid Application; Financial Theft; Fraud; Property Crime; Vandalism, Regulatory Non-Compliance; Safety Hazard; Ethical Misconduct; Policy Violation; or Other Concern.
Required: Enter where the incident occurred, or is expected to occur. Exact locations, such as a specific building, parking lot, or nearby area, help us respond effectively and keep our community safe. If the issue involves digital activity or physical mail (such as online fraud or scam mail), simply enter "Online" or "Mail."
Required: When did this occur or when was it discovered?
Required: Explain what happened or why you suspect a violation has occurred. If reporting identity theft/financial aid fraud, please include any reference numbers, student IDs, or document names from the mail/email you received.
Optional: If you do have any information about who may be involved, such as a suspect's name, email address, phone number, or website, please share it here. Otherwise, leave this blank.
