Records Transmittal Form

This form is to be used by offices with approved records retention and dispositions schedules that require transfer of records to the University Records Center. This form notifies staff of an impending records transfer and represents the official documentation of that transfer. Please document the shipment of only one record series per form, and please include a copy of each transmittal form in the first box of each series. To arrange transfer, please contact Central Receiving and submit a work order.
