MFA forms:
- Guidelines for Thesis Prospectus
- MFA Plan of Study - to be submitted by 1st year MFA students to Paul Lehmberg by November 15th.
- MFA Thesis Application - to be submitted by 2nd year MFA students to Paul Lehmberg along with prospectus and thesis credit registration by November 15th.
- Thesis Credit Registration (EN 599 & 699)
- Thesis Signature Approval Form - 2 originals are required

MA forms:

- MA Thesis Application - to be submitted to David Wood along with prospectus and thesis credit registration by
- end of fall semester for graduation the following fall or
- end of winter semester for graduation the following winter.
NOTE: Only a thesis chair needs to be chosen because the reader will be chosen as a collaboration between the student and their chair. - Thesis Credit Registration (EN 599 & 699)

Travel forms:
- Authorization for Travel (come to English office to pick up form) - basic form needed for any travel to be funded by NMU. This form needs to be turned in to English office BEFORE travel occurs.
- Special Projects Funding (pdf) - to be used to request moneys for travel from the English Department, College of Arts & Sciences, and School of Graduate Studies. Student must be presenting a paper to receive funding with this form.
Special Projects Funding (doc) - Student Travel Funds Application - to be used to request moneys for travel from the Student Travel Fund.
- ***NMU Travel Policy*** - Included as a reminder of reimbursement amounts including meals and mileage.
Miscellaneous forms:
- Application for Internship - Graduate Student Internships also need the Research/Practicum Credit Form
