To appeal your academic suspension or dismissal from Northern Michigan University, this form and all official documentation must be submitted at least five business days prior to the first day of the semester you would like to return in order for the Admissions and Academic Policies Committee (AAPC) to hear your appeal and reach a decision in a timely manner.

Please note this list is NOT all-inclusive. Its purpose is to simply provide examples. Although official documentation is required for your appeal, it does not guarantee that your appeal will be granted.
  • Illness – Letter from medical professional identifying patient, explaining medical condition, stating dates of illness and describing course of treatment
  • Death – Copy of obituary or death certificate
  • Legal issue – Copy of court report or letter from probation officer
  • Financial hardship – Copy of termination letter
