Dear New or Visiting Athlete, Prior to participation in sport at the USOEC you will need to complete medical paperwork for Sports Medicine. Your request will need to be accepted by staff. After accepted you will receive an email from SportsWare stating your request has been granted. Please follow email prompts to complete the paperwork. Fill out all the online sections My Info and Medical History. Please complete all 5 sections in My Info: General, Address, Emergency, Insurance and Medical. After entering all your information go into the “Forms” tab and select “MGH Consent Form”.Please print, complete, and bring when you arrive at the USOEC. Turn in the form along with a copy of your insurance form. Scanning the completed form and uploading on to SportsWare into the Forms tab is another option. If you have completed the online SportsWare information in the past you will need to log on with your username and enter your password. Then update any new information.
