The templates and forms that follow have been approved for use in all university-sponsored programs involving minors on campus. The program administrator may insert the name of the program and/or the sponsoring department near the top of the form. An approved academic or athletic logo or icon may also be inserted. The Risk Management department must approve any other changes.
