Q.  When is coverage effective?

A.  Coverage is effective on date of hire if enrollment form is completed within 30 days of hire.

Q.  What is the benefit year?

A.  For Blue Cross the benefit year is January 1 through December 31.

Q.  What if I have a pre-existing condition?

A.  There is no pre-existing clause.

Q.  How do I appeal a health plan decision?

A.  Please refer to the health plan appeals process.

Q.  If I waive coverage, is there any monetary benefit  in lieu of coverage?

A.  No.

Q.  Am I able to get a second opinion for a diagnosis or procedure?

A.  Yes.

Q.  Where and When do I get my Blue Cross Cards?

A.  Cards are mailed to your home address usually within a week of enrollment.  Extra cards may be ordered by you from the Blue Cross web site ( or through the Human Resources Office by calling 227-2470.

Q.  Why isn’t my spouse’s name on his/her Blue Cross card?

A.  All cards have the name of the contract holder (i.e. employee) on them.

Q.  Do I get a separate card for prescriptions?

A.  No.  Your Blue Cross card is also your prescription drug card.

Q.  When can I add someone to my health coverage?

A.  When there is a status change or during the annual open enrollment period (normally December each year).

Q.  Who do I call with questions about coverage?

A.  Call Blue Cross Blue Shield directly.  The customer assistance number is on the back of your card.

Q.  What is my group number?

A.  Your group number is 007004603.  It is also on your Blue Cross Card.

Q.  What is my contract number?

A.  Your contract number is the 9 digits found on your card.  The first three letters are your prefix.
