Testing Accommodation Form

One week prior to test is recommended to ensure that accommodations can be arranged.

Come to your test prepared.  If you are allowed a calculator, laptop, textbook, or notes of any kind, you are responsible for bringing them to the test.  Disability Services will not provide you with any of these.
