Warning or Academic Probation Agreement Form

This is an agreement between Northern Michigan University (“the University”) and you.  By signing this document, you acknowledge that you have been placed on probation by the University due to an Overall NMU Grade Point Average of less than 2.00 (the NMU Overall GPA only includes grades from NMU courses and does not include transfer or any other grades from non-NMU sources).  Academic Good Standing at NMU is a 2.00 Overall NMU GPA.  NOTE: “NMU GPA” denoted in the remainder of this document refers to the Overall NMU GPA described above.

This document refers to two types of probation for students whose NMU GPA is below 2.00.  Please read the following descriptions carefully and direct any questions to the Academic and Career Advisement Center (ACAC).

Warning Probation.  If your NMU GPA falls below 2.00, but is at least 1.70, you are placed on Warning Probation (WP).  While on WP you may not be Academically Suspended.  Depending upon subsequent academic performance and GPA, you will:

  • attain Academic Good Standing if your NMU GPA increases to 2.00 or higher,
  • remain on WP if your NMU GPA is between 1.99 and 1.70, or
  • be placed on Academic Probation if your NMU GPA falls below 1.70.

Academic Probation.  If your NMU GPA falls below 1.70, you are placed on Academic Probation (AP).  Depending upon subsequent academic performance and GPA, you will:

  • attain Academic Good Standing if your NMU GPA increases to 2.00 or higher;
  • be placed on WP if your NMU GPA increases to between 1.99 and 1.70;
  • remain on AP if your NMU GPA remains below 1.70, but you attain the required Semester GPA (see chart); or
  • be Academically Suspended if you fail to attain the required Semester GPA (see chart).

 *If enrolled in only one course, you must attain a 2.00 Semester GPA or higher (1.80 GPA can only be achieved through an average of grades from more than one course).

Once you attain/regain an overall NMU GPA of 2.00 or above (on a 4.00 scale), you will be released from probation and this agreement.

To assist you in attaining/regaining academic good standing, it is recommended that you:

Meet regularly with your assigned academic adviser to discuss various strategies for success.(Required)
Become familiar with, and utilize, campus resources designed to assist you in becoming a more proficient student.(Required)
Attend ALL class sessions – national research clearly identifies “lack of attendance” as the top reason why students are unsuccessful in college.(Required)
Communicate regularly with your instructors to have an accurate picture of your academic performance in each class.(Required)
Ask for help – start with your instructors, the tutoring center, ACAC, etc.(Required)
Read your NMU e-mail on a daily basis (at least).(Required)

By checking all six items and submitting this form, you indicate that you have read and understand all of the terms and conditions of this agreement and acknowledge that failure on your part to achieve the required semester GPA will result in suspension or dismissal from NMU. Once completed your registration hold will be lifted.
